Before you can have an appointment you must first register for clinic services via a Regstration Appointment.
- Picture ID
- Proof of residency (gas or electric utility bill, telephone bill, cell phone bill, rental/lease agreements, mail from state or local agencies not more than 30 days old)
- Proof of income (recent copy of your filed tax return, or two (2) recent and consecutive pay stubs, or social security earning statement)
- Insurance card, when applicable

Please note: Registration Appointments are not necessary for the following services: HIV Testing, Counseling and Testing Services and STD Clinics.

If you are experiencing a life-threatening emergency you are urged to call 911 or seek care at the nearest emergency room.
Contributing to your Care
As of September 2, 2014 we now offer services to both insured and uninsured patients.
If you have insurance you will pay the co-pays and deductibles your insurance policy requires. We accept most major insurance plans, including Medicare and Medicaid.
If you do not have insurance: we strive to provide affordable healthcare to everyone using a sliding fee-scale that is based on income We ask that you contribute to your care in some way, as patient donations are critical to the Clinic's success.

If you are unable to make it to your appointment, please call 816-753-5144, Option 3 to cancel. Missing three appointments without canceling will eliminate you from receiving Clinic care for up to six months.
Usually, it is possible to schedule a follow-up appointment as you check-out.
The Dental Clinic only schedules appointments for existing Clinic patients. If you are a patient at the Clinic, ask the doctor who sees you to refer you for a dental appointment. Click here for more information.
TB Testing: Services are offered on Wednesdays from 9–11 a.m. on a walk-in basis at our Broadway location. Patients must return on Friday morning between 9–11 a.m. to get their TB test reading. Patients who do not return for their reading on Friday must wait two weeks and be retested. At this time TB testing is available only for those who do not have health insurance. If you are insured and need a TB test, please contact your local health department.
